What to bring to your appointment.
- New Patients
- New Patient Paperwork
- Insurance Card
- Insurance co-payment or a deposit if you are a self-pay patient. (We accept cash, check, and all major credit cards. A $150 deposit for office visits is asked of all self pay patients. It will be applied to your total bill and any excess will be promptly refunded.)
- Any relevant tests, x-rays, or medical records.
- A list of all medications you are currently taking.
- Patients who have seen us in the past two years can omit the new patient paperwork. Please bring insurance cards and copayment to all appointments.
